Complaints and Grievances
Resolute Health Hospital wants you to be more than satisfied by the care you receive while staying at our facility. We strive to do our very best to assure you receive the best care we can possibly provide.
If at any point in time you are not satisfied by the care you are provided at Resolute Health Hospital please contact a Resolute Health Hospital representative and we will do everything we can to remedy the situation.
Concerns or Complaints
If you would like to voice a concern or complaint about any aspect of the care you received while visiting Resolute Health Hospital, please contact us at any time.
If for any reason, Resolute Health Hospital cannot resolve your concern you are also able to file a complaint with the federal government at the Office for Civil Rights of the U.S. Department of Health and Human Services regional office.
You have the right to decide what may be done to your body during the course of medical treatment. Your physician will discuss with you the nature of your condition, the proposed treatment and any alternative procedures that are available. Your physician will also provide you with information about the risks associated with certain medical procedures. This information will help you make an informed decision about the kind of treatment you want to receive.
Resolute Health Hospital is required by law to give you this Notice and to follow the terms of the Notice that is currently in effect. If you have any questions about this Notice, or have further questions about how the Hospital may use and disclose your medical information, please contact a Resolute Health Hospital representative.
