Follow My Health, the patient portal from Resolute Health, allows you to download and transmit your up-to-date health information. Log in from home, work or anywhere Internet access is available — 24 hours a day, 7 days a week. 

Follow My Health Patient Portal allows you to: 
  • Privately view some lab results online.
  • Access health records such as immunizations, medications and allergies.
  • Access health records to easily share with doctors.
In the future, we will expand the information you can access and enhance the portal’s features and functionality.
